Case Management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote patient safety, quality of care, and cost-effective outcomes.
We provide Case Management service for individuals on SCDDSN's Community Supports Waiver and Intellectually Disabled/Related Disabilities Waiver as well as other Case Management services.
There are two types of Case Management services available to DDSN eligible individuals who are not enrolled in a DDSN Waiver:
Medicaid Targeted Case Management (MTCM): Case Management services for client’s who currently are enrolled in Medicaid and meet the requirements for case management services
State-Funded Case Management (SFCM): Case Management services for client’s who currently are NOT enrolled in Medicaid and meet the requirements for case management services
