How do Our Service Coordinators Help You?

Service Coordination at Great Kids & Awesome Adults provides essential support to families by organizing and managing services for children and adults with developmental delays, disabilities, or special needs. Our goal is to simplify access to resources and ensure each individual receives the right support at the right time.
Our Service Coordinators work closely with families to identify their needs and coordinate a personalized plan, connecting them to necessary services, including:- • Therapies (physical, occupational, speech, etc.),
- • Medical referrals for ongoing care or specialized treatment,
- • Government-assisted programs such as Medicaid and Social Security benefits,
- • Community-based programs that offer additional resources and support.
Service Coordinators act as advocates, navigating the complexities of service systems to ensure families can focus on what matters most, their loved ones’ growth and well-being. This includes helping to develop and manage an Individualized Family Service Plan (IFSP) or an Individualized Support Plan (ISP), establishing clear goals and outcomes to track progress and adapt services as needed.
Families may select Great Kids & Awesome Adults as their Service Coordination Provider upon eligibility, empowering them to access the comprehensive support they deserve with ease and confidence.
