What are Case Management Services?

Case Management Services provide a collaborative process of assessing, planning, facilitating, coordinating care, and evaluating support options for individuals with disabilities and their families. These services advocate for access to healthcare and community resources to meet each individual’s unique needs, promoting safety, quality of care, and cost-effective outcomes.

Case Management services are provided by Waiver Case Management (WCM) for individuals enrolled in the Intellectual Disability and Related Disabilities Waiver (ID/RD) or the Community Services Waiver (CSW) or Medicaid Targeted Case Management (MTCM) for individuals enrolled in Medicaid or State-Funded Case Management (SFCM) for those not enrolled in Medicaid. Each type ensures that eligible individuals receive the support needed to reach their fullest potential.

Our dedicated Case Managers facilitate Service Coordination, connecting individuals with essential services such as medical referrals, therapy, and government programs. This coordination ensures a comprehensive approach to care, enhancing each person’s quality of life.

Case Managers work alongside families, developing a Case Management Support Plan (CMSP) tailored to the individual’s needs, goals, and family preferences. This CMSP serves as a guide for service delivery, focusing on the individual’s long-term success and well-being. Through regular meetings, Case Managers provide continuous support and guidance.

Upon eligibility, families may select Great Kids & Awesome Adults as their Case Management Provider, ensuring compassionate and effective care for those we serve.
